This is the first form that you need to fill out if you are seeking Disability Services. The information can be added and on this link and directly emailed to firstname.lastname@example.org . This will begin the process.
This form needs to be filled out by the professional that is diagnosing your disability. You do not need to have this form filled out if you already have appropriate documentation that follows the guidelines listed in the requirements link on the checklist tab on the left.
If one of your approved accommodations is to test in a separate location than you need to fill out this form, arrange through the DSC a time and place to take the exam then clear it through your professor by having them fill out their section of this form at least 3 days prior to your exam.

Agreement for Receiving Course Lecture Materials form
If one of your approved accommodations is to receive lecture notes from the professor, or through a note taker fill out this form, have it signed by the professor then deliver it to the DSC.
Release to receive medical information form
If for some reason you are unable to obtain appropriate documentation but feel that your doctor may have it, visit the Disability Services Coordinator ( Barber Hall 101D, or Caldwell campus office 128) and fill out this documentation (in triplicate). The DSC will then fax this request to your provider.
Seizure Response Plan
If you have been diagnosed with Epilepsy or any other disability that makes you prone to seizures please visit the Disability Services office (Barber Hall 101D, or Caldwell campus office 128) and fill out this documentation (in triplicate), to ensure your safety in case of an emergency.
Audio Recording Lecture Policy
If one of your approved accommodations is to record the class lectures please fill out this form, have your professor sign it then deliver it to the DSC.
Alternate textbook format policy
If one of your approved accommodations is to have your textbook in an alternate format (voice over, Braille, etc.) fill out this form and email it to the DSC at least 3 weeks before next quarter classes start.
Reader/Scribe testing accommodation agreement
If one of your approved accommodations is to have a designated scribe or reader for exams this form must be filled out and signed. It requires signatures from both the student and their support. This form is produced in triplicate.
