Disabilities Services Checklist
Now that you've decided to find out about Disability Services at TVCC-to ensure you are receiving every support possible-there are a few things that you need to do;
1. Request for Accommodations Form (below 1a)
Begin the process of filling out important documents to determining eligibility to receive a 504/Accommodation plan.
Eligibility under Section 504: An individual with a disability(s) is a person with a mental or physical impairment that substantially impairs or restricts one or more major life activities, such as caring for oneâ€™s self, performing manual tasks, walking, seeing, hearing, speaking, breathing, working, and learning. Reasonable, special accommodations are necessary for the individual to access and participate in her/his education.
2. Medical Documentation Form (below 1b)
Acquire mandatory documentation of the disability. Find the minimum documentation requirement article here.
Previous case reports for students on an IEP are accepted. An IEP by itself is useful but not adequate. Proper medical forms are accessible from the link above or on the Forms link. These must be filled out by a qualified professional, also outlined in the link above.
3. Contact Molli Vineyard to make an appointment for an eligibility meeting. This is when we determine eligibility for a 504/Accommodation plan based on all the forms and documents that you have filled out. Make sure to bring the medical documentation to your appointment. 4. Make sure to show up for this meeting!
We will determine what accommodations are necessary for student success.
Tip: The documents that will be generated in this meeting are important for success in your classes. It is vital that this process is completed before classes start. 504/Accommodation plans can be generated during the quarter. However, accommodations are not retroactive, so plan accordingly.
